The Fine Print

The VIP Membership Program is a dental discount plan; it is NOT dental insurance. The Membership is an affordable option for patients without a traditional insurance coverage or whom hold an insurance plan that is out of network with our office. The discount plan CANNOT be combined with any dental insurance, and is honored at all Smile Center Ohio Locations.

Should there be dental treatment needed following any type of injury where outside medical, car, disability, or workman's comp type insurance are involved, the VIP membership discounts cannot be utilized.

The patient understands that this plan is valid for ONE YEAR, which starts the day of enrollment, and ends the same day of the consecutive year. This plan is based on an individual basis, is non transferrable, non refundable, and is subject to change yearly.
